Ashamed to smile

I am a full time student desperately trying to get my degree in Early Childhood Education and Special Education.  I work while attending school, just to make ends meet.  I can't afford health insurance and I have thousands of dollars of dental work that needs to be done, including a cavity in my front tooth that makes me incredibly embarassed to smile.  To make matters worse, I have a deathly fear dentist.  My parents stopped taking me when I was 7 and had a panic attack in the chair and I was injured and the dentist yelled at me.  I was sexually abused for 9 years and I believe that I experience flashbacks while in the dental chair. The last dentist I went to asked me to leave because my tears were making him nervous.  I am a completely normal person in my real life, but the dentist just truly scares me.  I have dreams of going into politics and non profit work to change public education, yet I can't do that without being able to show the smile I rely on.  I am asking for any help or guidance that can help me to get over my fear and make me smile again.

I live in the Atlanta area.
